Through an array of personalized services, we strive to help every individual we support become increasingly independent and live life to the fullest in the communities they call home. Whether through residential programs that offer 24 hour support, our day program or hourly services delivered at home or in the community, Delaware MENTOR provides caring, goal-directed services that are designed to meet each person’s needs and preferences.
Building Relationships. Enriching Lives.
We specialize in supporting individuals with complex challenges and helping individuals transition out of more structured settings, such as hospitals and institutions, so they may return to their communities. Across our continuum of support options, we strive to help individuals live richer, fuller lives in the communities of their choice by:
- Improving access to community resources
- Participating in community activities
- Enjoying greater personal choice
- Developing meaningful personal relationships
- Developing independent living skills
Delaware MENTOR is licensed by the Division of Long Term Care and their Community Living Arrangements standards, which are monitored and certified by the Division of Developmental Disabilities Office of Quality Management.
