Hourly Support Services
Delaware MENTOR’s Hourly Support Services programs provide individualized supports and case management for adults with intellectual and developmental disabilities who live independently or with family members.
Our team of caring professionals provides a comprehensive assessment to identify each individual’s unique needs and goals and then works with them and members of their circle of support to develop a support plan to help them thrive. Depending on each individual’s needs, this may mean work, social and other community-based interests. We regularly re-evaluate each individual’s service plan to ensure it reflects his or her current goals and interests to help them keep moving forward.
Skill Development and Community Integration
To help each individual grow and be active in their community, the Hourly Support Services program is tailored to their unique needs and may include supports such as:
- Medication management
- Help with personal care
- Daily living skill development
- Health management
- Money management
- Grocery shopping
- Community activities
